# Generate ISRF

> Fill the FY2027 Individual Student Record Form from a student intake record.

**Daily → Generate ISRF** (`/admin/isrf`) fills the FY2027 Individual Student Record Form (ISRF) PDF from information already on the student record.

  **Admin**, **Data Lead**, and **Data Member** can generate ISRFs. Intake Members do not see this page.

## When to use it

Use this after a **NEW** registration (or after Data Leads complete demographics on the student record) when staff need a paper ISRF for the student file. Empty intake fields stay blank on the PDF for staff to complete by hand.

## Fill an ISRF

  <Step title="Open Generate ISRF">
    In the left sidebar, **Daily → Generate ISRF**, or go to [nycadultedlabels.nyc/admin/isrf](https://nycadultedlabels.nyc/admin/isrf).
  </Step>

  <Step title="Search">
    Type at least two characters. Search matches name, **Label ID**, **Student ID**, or date of birth.
  </Step>

  <Step title="Select the student">
    Click **Select** on the matching row. A field list shows what will be written onto the form.
  </Step>

  <Step title="Preview or download">
    **Preview** opens the filled PDF in the page. **Download** saves a file named like `ISRF-Last-First-LabelID.pdf`.

## What the PDF includes

When the intake record has the values, the form fills:

* Name (including middle initial), date of birth, gender
* Address
* Home and cell phones, email
* Emergency contact
* Original start date
* Employment status
* Hispanic / Latino origin and race / identity
* Barriers to learning / employment (Yes / No)

Date boxes on the form use **MMDDYYYY** (eight digits, no slashes).

<Note>
  **SSN** and the **student signature** stay blank. Staff complete those on the printed form.

Demographics are collected on **NEW** intake — see [Intake workflow](/workflows/intake#new-student-demographics). RETURNING visits do not re-ask those questions.
